For those seeking a more defined, curvaceous profile, the Brazilian Butt Lift (BBL) offers a unique solution that addresses two concerns at once. Using fat transfer, we can remove stubborn fat from unwanted areas and use it to add soft, natural volume to the buttocks. At our Paramus practice, Dr. Gary Breslow prioritizes safety and symmetry, ensuring your results enhance your proportions and fit your personal style.
Although the technique has been around for decades, the Brazilian Butt Lift (BBL) has been brought back into the limelight by the attention given to the world-famous derrieres of celebrities such as Kim Kardashian and Nicky Minaj.
The Brazilian Butt Lift is a form of fat grafting, in which fat is harvested from one area of the body and injected into another.
Pioneered in Brazil by world-renowned plastic surgeon Ivo Pitanguy, the procedure takes place in two stages:
Not only does the procedure lift and shape the buttocks, but removing unwanted fat from other areas of the body also helps shape and contour those areas

The BBL procedure usually takes two to three hours, depending on the amount of liposuction required. Anesthesia is used either locally on the affected areas or administered systemically. Although liposuction can cause some post-op discomfort, there is little to no pain from the fat injections.
In the first step of a BBL, small, thin tubes called “cannula” are inserted into tiny incisions in the skin and used to suction out excess fat. This fat is then processed and purified. In the second part of the procedure, the processed fat is reinjected multiple times into the buttocks at varying depths, drawing on the plastic surgeon’s skill and experience to ensure that the proper areas are targeted to achieve the desired shape as a final result. The transferred fat cells are then naturally integrated into the body in their new form.
Note that some fat cells do not survive the process, and that the projection of the buttocks may decrease during recovery. On average, patients retain 75% of the transferred fat. This is perfectly normal and part of the healing process.
With qualified, board-certified plastic surgeons, medical personnel, and the proper equipment and techniques, there are usually few medical complications apart from the usual risks associated with surgery.
The Brazilian Butt Lift procedure is ideal for addressing a flat or square-shaped posterior that may be due to genetics, aging, or weight loss. It is also an excellent solution for correcting asymmetry or "hip dips" that exercise alone cannot fill. By removing fat from the midsection and redistributing it to the gluteal region, Dr. Breslow improves the overall balance of the torso, creating a more youthful, toned appearance.

While the results of a Brazilian Butt Lift are most apparent in the buttocks, the BBL is actually a multi-area procedure. We address the donor sites—typically the waist, back, and thighs—to reveal a more tapered midsection. This "top-to-bottom" refinement ensures that your new curves are the focal point of a balanced, athletic frame.

The Brazilian Butt Lift is a cosmetic surgery procedure that transfers excess fat to the butt from other parts of the body, leaving a sculpted, slimmer waist and a fuller, rounder butt. Sculptra, on the other hand, is a non-surgical procedure that relies on injections of a dermal filler in targeted areas to enhance and shape the buttocks.
Both approaches offer the following benefits:
The most significant benefit of a BBL is the use of your own natural tissue, which eliminates the risks associated with synthetic implants. The results feel soft and move naturally with your body. Many patients report that their clothes fit better and they feel a newfound confidence in swimwear and athletic apparel. Beyond the physical volume, the liposuction performed during the procedure provides a level of waist definition that exercise alone often cannot achieve.
Candidates for a Brazilian Butt Lift must be in good overall health and have sufficient excess body fat to be removed and re-injected. The ideal candidate for this procedure is generally under 50 and has a BMI under 35.
Patients who lack sufficient excess fat for transfer may opt for Sculptra injections instead.
Having performed butt enhancement procedures on countless patients, covering a wide range of body types and aesthetic goals, Dr. Breslow, our board-certified plastic surgeon, is among the most experienced BBL providers in New Jersey. Feel free to discuss your questions and expectations in detail with him during your consultation appointment at our office before planning the procedure.
An hour or two of observation is typical, after which the patient can return home with a compression band, which remains for about a week
The first week after the procedure should focus on healing, with special precautions when sitting or lying down to minimize disruption and discomfort. Two to three weeks after the procedure, patients should be able to resume most activities, such as walking and less strenuous exercise, while still paying special attention when sitting or lying down.
Some swelling is to be expected in the first few weeks. After 3-6 months, all areas should be healed, revealing the buttocks’ final shape and size. Supervision and feedback from the plastic surgeon is important to ensuring a healthy and stress-free transition back to normal activities.

When can I see results?
You will see an immediate change in your silhouette following surgery. However, there is a "settling" period where some of the transferred fat is naturally absorbed by the body. Your final, permanent shape will be visible after three to four months once the swelling has fully dissipated.
How long do results last?
The fat cells that successfully integrate into their new location remain. Your results are intended to be permanent, though they will still respond to natural weight fluctuations. Maintaining a healthy lifestyle and a stable weight will ensure your new curves remain defined and beautiful for years to come.

Yes, and growing in popularity. Brazilian Butt Lift procedures increased by 80% between 2000 and 2013, according to the American Society of Plastic Surgeons.
Common side effects include bruising, swelling, pain, irregularity, asymmetry, infection, and fat loss.
This varies from person to person, depending on how the transplanted fat cells are reabsorbed by the body, but generally lasts for years or even decades. Subsequent weight loss and gain may cause sagging later on.
Again, this will vary, but generally between 300 and 600 cubic centimeters is required. Patients should be roughly 10-15 pounds over their ideal weight.
Yes, most surgeons consider a fat transfer safer than synthetic implants because it uses your own tissue, eliminating the risk of rejection or implant displacement.
If you have a very low body fat percentage, Dr. Breslow may discuss alternative options like Sculptra injections or suggest a different approach to body contouring during your consultation.
Generally, you should avoid sitting directly on your buttocks for at least two to three weeks, and using a specialized pillow when sitting is necessary.
We recommend waiting about six weeks before resuming strenuous lower-body workouts to give the fat cells the best chance to survive.
The incisions for liposuction are extremely small—usually only a few millimeters—and are placed in discreet locations where they fade significantly over time.
